Guilds, unions, societies, and federations: Exploring the different forms of doctors’ organizations

What is a Doctors Organization?

The Doctors’ Organization is an international medical organization founded to promote high standards of Medical Education, Medical Science, Medical Art and Medical Ethics. It also promotes the freedom of physicians to practice their profession worldwide.

Doctors’ organizations have struggled to resist the spread of electronic medical records, which turn them into data-entry clerks. Yet they have failed to organize a movement like that of the Parkland students.

It is a trade guild

Unlike unions, which focus on bargaining between workers and employers, guilds facilitate negotiations between their own members. These negotiations may embody some of the traits of union forms of democratic group decision-making. Guilds also set standards for things like general best practices, quality, safety requirements, and standard rates.

In the past, guilds provided charity and social support to their members. They also acted as a source of legitimacy, with some guilds having royal patronage. For example, surgeons in London belonged to the Barber’s Company and gained political influence through their large number and their status as an established guild.

Today, doctors can form a similar organization that will focus on the needs of patients. Such an organization could tackle the profound changes on the horizon for the medical profession. It might challenge anti-vaxxers; decry predatory, unregulated stem-cell clinics; and promote awareness of the health hazards of climate change. It could even free doctors from the tyranny of the keyboard by synthesizing patient data and speeding chart review.

It is a union

Considering the long-standing structure of medical residencies, the ticking-clock pressures of patient care and the fact that many young doctors are part of what polls show is one of the most pro-union generations in decades, it seems surprising that unionization among residents has been lagging. After all, the health care industry has seen nurses, custodial staff, and other workers organize in droves over the past decade and achieve major gains.

As a result, some hospitals are now pushing back, trying to discourage residents from joining unions. But that may be a mistake. Recruiting and training new doctors is an important task, and one that requires a stable work environment.

Besides, the UAPD’s Weinmann doesn’t buy the argument that joining a union makes physicians greedy or that the public will view them as more self-interested. The UAPD’s professional union negotiators are familiar with the issues facing salaried physicians, and they can help negotiate contracts that meet the needs of each group.

It is a society

A doctors’ organization is a professional society that unites physicians to achieve the highest standards of Medical Education, Medical Science, Medical Art and Medical Ethics. It disseminates health and scientific information to patients, doctors, and the public, and advocates for its own position in national and international governmental bodies and agencies. It also helps to set standards for medical schools and internship programs, and tries to detect and alert the public to quack medical remedies and charlatans.

It’s possible to imagine a doctors’ organization that has nothing to do with protecting trade guild interests, but everything to do with restoring the doctor-patient relationship. Such an organization could confront the transformative challenges that the profession is facing adroitly, instead of devoting so much time to administrative tasks that only thirteen per cent of doctors’ days are spent on patient care. It would have strong physician leadership, and a culture of clear, shared aims that enable meaningful performance measurement.

It is a federation

A doctors organization is a non-profit with a governance structure that enables the members to vote on strategic directions and ventures. Its primary mission is to serve humanity by promoting the science and art of medicine, protecting public health and encouraging the best in medical education. It also works for high standards of ethical behaviour and professional competence.

The organization provides confidential assessment, referral to treatment, resources and monitoring for physicians/healthcare professionals and medical students who may be impaired due to mental illness or substance use. This can be an important step in ensuring safe patient care and returning the physician to practice when they are ready.

The federation includes 116 national associations of doctors and their allied organizations. It promotes the interests of its members by educating and informing the public about medicine, health and related issues, and alerting them to quacks and charlatans. It also monitors and lobbies for legislation that affects physicians.
